Transfusion of Blood Products in the Critically Ill

Objectives

1. Review the current evidence and rationale for using a restrictive red blood cell (RBC) transfusion strategy in critically ill adults.
2. Identify patient populations where a restrictive RBC transfusion strategy may not be appropriate. Discuss the nuanced recommendations for subgroups such as patients with acute coronary syndrome (ACS), postoperative cardiac surgery patients, and those with isolated troponin elevations.
3. Interpret the risks and benefits associated with RBC transfusions across ICU subpopulations. Analyze outcomes including mortality, length of stay, and adverse events in patients with GI bleeding, septic shock, and other critical conditions.
4. Apply guideline-based RBC transfusion thresholds to real-world ICU cases. Emphasize the importance of clinical context, including hemodynamic status, organ perfusion, and patient comorbidities, when making transfusion decisions.
5. Promote evidence-based transfusion practices to optimize patient outcomes and healthcare resource use. Highlight the cost, safety, and implementation implications of a restrictive transfusion strategy across varied ICU settings.
