Objectives

1. Delineate the Overlap between new requirements for Value-Based Payments (MIPS) and the Patient-Centered Medical
Home (PCMH)

2. Recognize how the PCMH Model of Care can improve quality of care delivered to seniors, promote patient safety,
especially around transitions of care, and advance care information.

3. Understand the role of primary care physicians, subspecialists and hospitalists in assuring success of the PCMH model
