Description

We will be discussing a 63-year-old male with right sided paresthesia and facial droop

Objectives

    Identify vascular territories (e.g., MCA cortical branches, deep perforators causing lacunar infarcts, pontine perforators) that can produce right-sided paresthesia with facial droop. Distinguish cortical versus subcortical stroke patterns based on clinical presentation.

    Review evidence-based imaging pathways in vascular neurology, including CT/CTA, MRI-DWI, and perfusion imaging for hemisensory–facial syndromes. Incorporate vascular risk assessment and stroke mechanism considerations (small vessel disease, embolic sources, large vessel occlusion).

    Discuss thrombolysis and thrombectomy decision-making for presentations with mild or predominantly sensory deficits. Evaluate early secondary prevention strategies (antiplatelet therapy, BP management, statins) based on probable vascular mechanism
