We will be discussing a 48-year-old with confusion and rhabdomyolysis

Objectives

    • Differentiate neurovascular etiologies of acute confusion in the setting of rhabdomyolysis, including ischemic stroke, cerebral venous sinus thrombosis, hypertensive encephalopathy, and global hypoxic-ischemic injury, and recognize clinical and imaging features that help distinguish primary vascular causes from toxic-metabolic encephalopathies.
    • Examine the bidirectional relationship between rhabdomyolysis and neurovascular injury, including how prolonged immobilization, seizures, drug-induced vasculopathy, hyperthermia, or ischemic stroke can precipitate rhabdomyolysis, and how metabolic derangements (e.g., acute kidney injury, electrolyte abnormalities) can worsen cerebral perfusion and neurologic outcomes.
    • Develop a multidisciplinary diagnostic and management strategy for patients with altered mental status and suspected neurovascular disease complicated by rhabdomyolysis, emphasizing timely neuroimaging, laboratory evaluation, blood pressure and volume management, renal protection, and coordination between neurology, neurosurgery, critical care, and nursing teams to optimize neurologic recovery and prevent secondary brain injury.
