Description

Liver Transplant for Malignant Diseases

Dates and Times

Start: 10/21/2026 8:00 AM
End: 10/21/2026 9:00 AM

Objectives

Apply selection and staging criteria for HCC. Compare the Milan, UCSF, and Kyoto criteria. Explain how downstaging, AFP trends, and response to locoregional therapy (or immune checkpoint inhibitors) affect transplant candidacy and waitlist dropout.

Describe how HCC is allocated, and how recurrence is managed after transplant. Explain how MELD exception points and the mandatory 6-month wait work. Outline surveillance after transplant (imaging and AFP) and the treatment options for recurrence, including resection, mTOR-based immunosuppression, and sorafenib.

Weigh liver transplant against resection for small, resectable HCC. Use recurrence-free survival and long-term survival data. Consider organ scarcity and underlying cirrhosis, and the role of salvage transplantation.

Evaluate the expanding indications for transplant in non-HCC malignancy. Summarize the outcomes data for unresectable colorectal liver metastases (Norway, and North American living-donor data) and for neuroendocrine liver metastases. Discuss selection criteria, tumor biology, and allocation justice.
