Objectives

  1. Review that stroke care and recovery is a national problem which is projected to get worse.
  2. The Stroke Recovery Program takes a proven model used in cardiac rehabilitation and improves the healthcare delivery model for stroke survivors.
  3. Review outcomes of the Stroke Recovery Program showing that it benefits function, mortality, rehospitalization and cost.
  4. Address that stroke post-acute care deserves the same healthcare delivery options that we know works extremely well for the cardiac rehabilitation population.
