Description

Patients with chronic and life-limiting illness, the elderly, and the bereaved are commonly seen in primary care practice and the skill required to care for these patients are informed by an understanding of the principles and practice of palliative care.  This symposium focuses on providing education on the palliative care philosophy and how it can be incorporated into clinical practice at the primary care level.

Objectives

Patient Care
• Understanding and addressing the experience of physical, psychological, and spiritual suffering in specific patient populations
• Demonstrate the knowledge and ability to counsel patients and families on the indications and contraindications for invasive procedures, such as artificial nutrition, mechanical ventilation, withdrawal of care
• Understanding ethical issues in end-of-life care and respecting patients’ personal values
• Recognizing and responding to cultural, linguistic, and spiritual diversity and to varied personal styles

Medical Knowledge
• Understanding the pathophysiology and illness trajectories of specific chronic conditions
• Skillfully managing pain and other common, distressing symptoms commonly occurring in end-stage disease

Interpersonal and Communication
• Communicating effectively and humanely with the patient and family about diagnosis, prognosis, treatment options, and goals of care
• Working with an interdisciplinary team to provide comprehensive, coordinated care to optimize quality of life using a range of both aggressive and palliative care options

Professionalism
• Developing an awareness of one’s own attitudes, feelings, and expectations regarding death and loss
• Consistently demonstrates ethical conduct and emotional maturity

Practice Based Learning and Improvement
• Developing skills to evaluate and implement quality improvement projects related to patient care and safety
• Working with the interdisciplinary team to enhance the patient care experience and outcomes

Systems Based Practice
• Understanding the ethical and legal implications present in the practice of palliative medicine
