Description

Ophthalmology Clinical Rounds consist of a 1.5 hour lecture time by faculty and invited speakers who focus on new advances in the clinical and basic science fields of Ophthalmology. Emphasis is placed on contemporary techniques of diagnosis and medical/surgical therapy. The sessions provide a forum for group discussion of difficult or interesting diagnostic/management problems encountered by the specialist.

Objectives

1. Define neuropathic corneal pain and distinguish it from nociceptive ocular surface pain associated with dry eye disease.

2. Recognize key clinical features and symptom patterns suggestive of neuropathic corneal pain, including disproportionate pain, photophobia, and poor response to conventional topical therapies.

3. Apply a stepwise diagnostic approach to neuropathic corneal pain, incorporating symptom profiling, topical anesthetic challenge, functional sensory testing, and in vivo confocal microscopy.

4. Interpret functional and structural corneal nerve assessments, including hyperosmolar saline challenge, esthesiometry findings, and in vivo confocal microscopy features such as microneuromas.

5. Differentiate peripheral from centrally mediated neuropathic pain mechanisms to guide individualized management strategies.

6. Integrate evidence-based treatment options for neuropathic corneal pain, including neuro-regenerative therapies, neurostimulation approaches, and systemic neuromodulatory treatments.

7. Identify systemic associations, such as small fiber neuropathy, that may coexist with neuropathic corneal pain and influence multidisciplinary evaluation and management.
